Sample Differentiated Lesson - Using video, QR Code, Google Form

Here is a sample lesson part of a unit I have prepared.  Apart from the lesson plan there is also PDF activity sheet. The following page has an embedded copy of the Google form I have designed as a revision quiz. There is also an Google form to accompany the Fluent U video. Just for an option, I have added the QR code to access the link to the Fluent U video.
